Speech and Language Questionaire

If you can answer ‘yes’ to 4 of these questions, you should seek an evaluation by a speech pathologist.

Area of Interest: Articulation/Oral Motor
  • Do you find yourself interpreting for your child the majority of the time?
  • If he/she is older than 4 years of age, does he/she have trouble producing the r, ch, sh, l sounds?
  • Does your child have difficulty imitating tongue movement?
  • If your child is over the age of 2 years, does your child crave chewing common objects, other than teethers?
  • Is your child a picky eater?
  • Does your child stongly choose one type of food texture over another?
  • Does your child have strong food preferences that seem to be getting stronger?

If you can answer ‘yes’ to at least 4 questions, you should seek a consult with a speech pathologist.

Area of Interest: Language

  • By age 2, can your child have more difficulty than other kids their age expressing wants/needs?
  • Does your child avoid eye contact?
  • Does your child use grunting/gestures only to communicate wants/needs?
  • Are you and/or other family members frustrated by communication attempts?
  • Is your child easily frustrated failed communication attempts?

If you can answer ‘no’ to at least 3 questions, you should seek a consult with a speech pathologist.

  • By age 1 1/2, does your child use at least 5-30 words, even if they are not understood easily by everyone?
  • By age 2, does your child use 2-3 word sentences?
  • By age 2, can your child identify pictures in a book?
  • Does your child enjoy playing with one toy more than others?
  • By age 2, can your child follow one step commands fairly appropriately?
  • Does your child answer routine questions accurately?
  • Does your child use his own words to answering a question instead of repeating other sentences he/she hears?
  • Does your child use words more than gestures?
  • Does your child enjoy interacting with other children?
  • Does your child recall letters/numbers far quicker than others his age?
  • Does your child relate past experiences to present situations? (They are able to talk about what he/she did earlier in the day or last night.)
  • Does your child put information in a logical sequence?
  • Does your child find solutions to simple problems?
  • Can your child tell an imaginary story?
